I hereby give permission to Beau Bella Photography to use my photos and likeness in all forms and media for advertising, portfolio, demo, trade, stock photography, editorial, altering without restrictions, and all other lawful purposes. I understand I am entitled to no compensation. I release the photographer of all forms of claims and liability related to my photo usage. I am over 18 years of age, and I fully understand the terms of this release.

By Definition:
PHOTOGRAPHER means Beau Bella Photography LLC, Steamboat Springs, Co, and it's owners, operators, employees, successors, heirs or assigns and includes but is not limited to: photographer, illustrator, filmmaker or cinematographer, or any other person or entity photographing or recording me.
MODEL means me and includes my appearance, likeness and form, whether or not it is altered to be unrecognizable as me.
MEDIA means all media including digital, electronic, print, television, film and other media now known or to be invented later.
ASSIGNS means a person or any company to whom Photographer has assigned, transferred or licensed rights under this release as well as the licensees and/or transferees of any such person or company.
IMAGES means all photographs, film digital images, video or electronic recording taken of me as part of the photographic shoot.
CONSIDERATION means something of value I have received in exchange for the rights granted by me in this release. It may include but not be limited to copies of prints, the opportunity to be photographed, or something else of value, tangible or intangible.
SHOOT means the photographic or film session described in this form.
PARENT means the parent and/or legal guardian of the Model.
For good and valuable Consideration, receipt of which is hereby acknowledged, I grant to Photographer and his or her heirs, successors and assigns my permission to license, use reproduce and distribute images of me and/or my property in any Media for any purpose (except pornographic or defamatory) in which may include, among others, advertising, promotion, marketing and packaging for any product or service. I agree that the Images may be combined with other images, text and graphics, and cropped, altered or modified. By signing below, I hereby release, acquit, hold harmless and discharge the Photographer and his or her Assigns of and from any and all actions, causes of action, claims, demands, and lawsuits arising from the use of my name, fictional name and images of me and my property, in all forms and in all media and in all manners, without any restriction as to changes or alterations (including any but not limited to composite or distorted representations or derivative works made in any medium) for advertising, trade promotion, exhibition, or any other lawful purposes, and I waive any right to inspect or approve the Media or finished version(s) incorporating the Media, including written copy that may be created and appear in connection therewith. (To be specific, this includes: all Social Media: images will be used on a website, blog, Facebook, Pinterest, Instagram, etc.) I agree that I have no rights to the Images, and all rights to the Images belong to the Photographer and his or her heirs, successors or assigns. I acknowledge and agree that I have no further right to additional Consideration, remuneration or accounting, and that I will make no further claim for any reason to Photographer or it's Assigns. I acknowledge and agree that this release is binding upon my heirs and assigns. I agree that this release is irrevocable, worldwide and perpetual, and will be governed by the laws of the State of Colorado, USA. In the event that I ever bring an unsuccessful action against Photographer arising out of the shoot, I agree to be responsible for Photographer's attorney's fees, costs, and expert witness fees.

I represent and warrant that I am at least 18 years of age and have the full legal capacity to execute this release.
